Introduction: Every member of the clinical microsystem who provides and contributes to the care and safety of patients should be thought of as a professional. If every person in every role is respected for their contribution to the smooth functioning of the microsystem, then individual self-esteem, morale, and engagement all increase. This leads to better outcomes and higher satisfaction rates for the patients entering the clinical microsystem. Learning more about *all* microsystem professionals and what they do, what hours they work, what they wish to learn, and how they rate/perceive their work environment increases the nurse manager’s awareness for future improvements.

Clinical Microsystem Assessment #4: PROFESSIONALS. (Minimum 2 pages; Maximum 4 pages; APA 7th ed. format). You may need to meet with your Unit Manager to obtain all of the information required in each section. If you are unable to get the information in some categories, explain how not having this information poses challenges in your ability to assess this topic and its implications for identifying potential areas for improving your clinical microsystem. The points can be re-earned by including this information in the write-up of the final project.

Organize the paper using the following headings: Introduction, Current Staffing, Admitting Medical Service, Supporting Departments (Diagnostic, Therapeutic, Pharmaceutical), Staff Satisfaction, and Strengths and Weakness of the Professionals Assessment. Here are the areas that should be assessed in each section:

I. Introduction. In one paragraph, introduce your clinical microsystem and why the Professionals Assessment is important to improving patient outcomes. Use a minimum of 2 citations from the textbook and/or module readings to support the introductory content.

II. Current Staffing For each category below, indicate the number of FTEs (Full-Time Equivalents for Day Shift, Evening Shift, Night Shift (or 7am-7pm and 7pm-7am), Weekends, and any staff who receive Overtime based on their role. Introduce the results in 1-2 paragraphs and then include an at-a-glance Table to illustrate the data collected:

Discuss the diversity among professionals working in the clinical microsystem (ethnicity, gender, age, etc.). Consider if the diversity of the professionals mirrors the diversity of patients treated in the clinical microsystem. How might this impact care/outcomes?

III. Admitting Medical Service (%) In one paragraph, indicate the percentage of patients admitted through the following Medical Services: Internal Medicine, Hematology/Oncology, Pulmonary, Family Practice, ICU/CCU, Cardiology, ER, Other.

IV. Supporting Departments (Diagnostic, Therapeutic, Pharmaceutical): In one paragraph, briefly explain which departments not located within the clinical microsystem, but directly support patient care or treatment? (Examples include Respiratory, Cardiology, Pulmonary, Radiology, Physical Therapy, Psychiatry, Pharmacy, Lab, IV therapy, Clergy…)

V. Staff Satisfaction: Your perception of co-worker’s job satisfaction (%). As you are comfortable, talk to your co-workers at all levels to get a sense of how happy they are with their jobs in order to address these two issues (in one paragraph):

How stressful is the unit? (Indicate your perception of the total % Not Satisfied.) Are some categories of professionals satisfied, but others are not? If so, explain the differences in this section.

Would you recommend this unit as a good place to work? (Indicate your perception of the total % that Strongly Agree). Do some categories of professionals on your unit Strongly Disagree? If so, explain the differences in this section.

VI. Strengths and Weakness in the Professionals Assessment. Use two current (within the last 5 years), peer-reviewed NURSING journals to support the Strengths and Weaknesses content. In 1-2 paragraphs, summarize the complex issues related to staffing in your clinical microsystem. Identify one staffing-related strength and one area needing improvement in your clinical microsystem. Remember to consider all the categories of staff assessed in this module. Reflect on who does what when? Is the right person doing the right activity? Are roles being optimized? Is there anyone who is contributing to patient care and safety who is not listed in the table? Does the interface with supporting departments flow smoothly and enhance patient outcomes and safety?
