Follow these instructions:

Jim frequently argues with his family members and occasionally with friends and co-workers. Jim has never been able to maintain a monogamous relationship. He is addicted to cybersex and admits to currently cheating on his girlfriend. He reports difficulty with his sleeping, sometimes sleeping too much and sometimes not being able to fall asleep. He frequently goes driving in the middle of the night, when he is unable to sleep. He reports increased difficulty with concentration at work. He has admitted to cutting himself previously and he currently has scars. He has been on and off of prescribed medications and in and out of therapy for the past few years.

The textbook chapters this week address multiple treatment methods for infidelity and include specific case examples and treatment considerations. This week, you will read the provided vignette and propose a treatment plan.

Be sure to approach the following vignette by using the text concepts and your reflection and understanding of the phenomenon presented. Your writing should illustrate knowledge of the concepts through an original personal and/or professional integration of the text material. It is permissible to paraphrase or quote the text, but you must reference the author and indicate the page number. You are expected to use the resources listed under each activity for your work in this section, but you are also encouraged to seek other sources, including peer-reviewed research articles and websites. As vignette application is required, please reference the people and situations within the context of your answers. All answers must consider legal and ethical considerations, as well as issues related to culture and human diversity.

Pam is a 45-year-old bisexual Caucasian female, who began counseling due to her sex and drug addiction. She wants to find a way to deal with her feelings of sadness, difficulty sleeping, anxiety, difficulty concentrating, rumination, thoughts of hurting herself, and nightmares. Pam attributes her symptoms to the recent emergence of some repressed memories. Pam describes the memories as “coming out in my dreams” of her father sexually abusing her, when she was approximately 10 years old. Pam appears anxious, uncomfortable, and tearful as she describes the memories of the trauma. She reports she has been struggling with this for the past 2 months. Pam has been in a relationship for 1 year with a woman of Middle Eastern descent, and has been unfaithful. She feels guilty for cheating on her partner and would like to stop having sexual affairs. Pam reports not having any children nor having an interest in starting a family.

Pam received her master’s degree in film and television from a highly recognized university at the age of 35. She describes feeling frustrated with not being able to find employment in her field of study. She currently works for a major film and television studio in the accounting department and has been employed there for nearly 6 years. Pam describes the work as “boring and unfulfilling,” but does not have the motivation to look for employment in her field of study.

Prior to her current therapy, Pam reports receiving services at the USC Neuro-Psychiatric Center for five sessions due to symptoms of anxiety, depression, difficulty concentrating, and nightmares; but because her medical insurance ran out, she was unable to continue. Pam currently receives therapy two times per week. She denies being hospitalized in the past even though she has had thoughts of hurting herself.

Pam reports that she has one sister 8 years younger than she, who is married with a 2-year-old daughter. Pam also states that she had a younger brother, two years her junior, who died from cancer when Pam was 20 years old. Pam states that her parents divorced shortly after the death of her brother. Pam describes her mother as “encouraging” in regards to her work and academic performance but also “controlling.” She describes her father as distant, unemotional, and expressing love through monetary means. Pam describes her parents’ relationship as tumultuous with “a lot of yelling.” She states that when her mother would get angry with her, she would yell and “dig her nails into her arm” and her father would withdraw and avoid all contact with her. She states that prior to the emergence of her memories of abuse, she would visit with her parents approximately one time per month. Pam reports she has not made any contact with her father since the recall of her memories. She describes her relationship with her sister to be distant, but cordial. She reports she visits with her sister approximately two times a month. Pam describes her mother as “always appearing depressed” and states her sister has been struggling with an alcohol addiction for many years.

Pam is currently married and describes her relationship with her wife as difficult at times. With further prompting, she states that the cultural differences between her and her wife create challenges in their communicating with one another.

Pam denies any medical problems. Pam reports she smokes marijuana at least two times per day, and started doing so at the age of 13. She reports using cocaine once or twice a week at a party setting, as well as having sex with multiple partners while under the influence. She reports drinking two to three glasses of wine each night as a way of “relaxing and taking the edge off.”

  • Theories of sexual compulsion have pointed to a variety of other factors, including co-morbid diagnosis of other disorders such bi-polar depression, obsessive-compulsive disorder, and ADHD as underlying them. With the information presented for Pam, what other disorders might he also be dealing with?
  • What steps would you take in initiating treatment for Pam?
  • Discuss the treatment options for Pam to address infidelity. Which mode of treatment (individual, group, or couple’s therapy) would you recommend for Pam? What short and long-term goals would you set up in the treatment plan?

This study compared West Point cadets (N = 103), Norwegian Naval Academy cadets (N = 141), and U.S. civilians aged 18 to 21 (N = 838) with respect to 24 character strengths. Results generally showed that the absolute scores of West Point cadets were higher than either of the other 2 groups. However, when the rank orders of character strengths were compared, the 2 military samples were more highly correlated with each other than either was with the U.S. civilian sample. The greatest strengths evident among the military samples were honesty, hope, bravery, industry, and teamwork. Implications for multiforce military operations are discussed. Case Vignette 

A Caucasian male OEF, OIF veteran (30) presents with his wife (27) and young son (7) for counseling. Both the veteran (“Vet”) and his wife complain about feeling disconnected from one another, and Vet complains about feeling no emotion toward his son. Upon inquiry, Vet described a traumatic event during his second deployment. Vet reported that during a routine search and clearance, a young Afghan boy was standing in the middle of the road. Vet described his attempts to get the young boy to move; however, his attempts failed. When the young boy did finally move on his own, it was clear that he had an explosive. It was then up to Vet to make the decision to shoot the young boy. Vet described nightmares and frequent flashbacks about this event.

Vet’s wife reported that she no longer knows her husband. She reported that he fights in his sleep, and they have not had sex in more than 10 months. Wife stated that she is unhappy in the marriage and is disappointed with how her husband treats their son. She described her husband as a monster, and stated that she is fearful, but not physically afraid for the family. She reported that there are weapons in the house and Vet’s temper is easily triggered. She hopes that Vet can open up to her more regarding his experience because this was the first time she’s heard about his trauma.

Vet’s son presented as quiet and solemn. He made little eye contact and looked to his mom when questioned about his father. His grades are starting to slip in school, and teachers are concerned about his emotional well-being. Teachers reported that Son tends to hold his head down and takes correction more harshly than before. Son suggested to friends that he does not want to be at home. He received several bullying attacks but refused to provide administrators with their names.

There are times that the family enjoyed being with one another. They reported playing video games and paintball together. They are of Catholic faith but do not attend regularly. They live close to the wife’s family and frequently attend family functions. There are no financial issues. Wife and Son have no known health or medical issues. Vet is not on any medication and has not sought therapy in the past. The family’s goal is to be closer and happy.

Assignment

Read the vignette and discuss what method of therapy would be most useful for this case and why. Please include five interventions associated with your chosen model and describe implementation of these interventions to the issues presented in the vignette. Be sure to include a treatment plan. To support your model choice and proposed interventions, provide at least five references from the NCU library in addition to the materials covered in this course.

Consider how your own values, beliefs, and assumptions will guide your approach toward couples and sex therapy while providing a crisis intervention. Choose one of the following scenarios (please pick a scenario that feels challenging to you):

  • Early in the second therapy session one partner announces that she/he is leaving the marriage and walks out of the session.
  • Several weeks into therapy, the husband announces that he always felt an attraction to men and now believes that he cannot live his life as a lie and ignore these feelings. While he still respects his current spouse, he would like to work towards dissolution of the marriage.
  • A couple comes to therapy because the male/female partner wants to bring a third person into their relationship.
  • A couple comes to therapy with polarized views on having kids. One partner wants children and the other does not. Would your views about this issue change if the couple consists of different sexes, the same sex, older (in their late forties), or younger (mid-twenties)?
  • After attending therapy for five sessions, one partner tells you (in private) that she/he had an affair. This partner wants to continue to work on the marriage and insists that you not tell the other partner.
  • You are working with a couple in their mid-twenties who have been married for just over a year. They came to you because they are having sexual problems. They report having had frequent and mutually satisfying sex before they were married; however, once married, the amount of sexual encounters decreased dramatically. Upon further exploration of their history, you learn that they have been attending a local church marriage enrichment class. One partner admitted feeling significant shame about having had premarital sex. This partner feels that had they waited, their sexual experiences would be more fulfilling currently.
  • A husband and wife come in to work on a sexual issue. About 4 weeks into therapy you learn that one of the partners is only attracted to people of his/her same sex yet they are committed, because of religious beliefs, to stay in a heterosexual relationship.

As you consider working with the couple from your chosen scenario, write a discussion post addressing the following:

  1. Which scenario did you choose?
  2. What might you be feeling?
  3. What values do you hold about the issue(s) your clients are working through?
  4. How will you manage your feelings to be of help to the couple?
  5. How will you approach therapy from this point forward?
  6. What steps will you take to continue to explore these and other values?
  7. Identify at two resources (at least one being a peer-reviewed journal article) either from your readings or your own library search that will help you further explore this topic.

You are welcome to use any of the models of couple therapy that are addressed in this course or if there is a different model of therapy that you feel more accurately reflects your theory of therapy, please email your professor to obtain permission to use it for this assignment.

In addition to explaining and critiquing your chosen model, you’ll apply this model to a specific case. If you are currently working with couples in therapy, you can use one of your cases as the focus for this assignment. Alternatively, please develop a case study from a movie from the following list:

  • The Notebook (2004)
  • The Thing about my Folks (2005)
  • Family Man (2000)
  • Away from Her (2007)
  • My Life (1994)
  • Contact your professor to get an alternative movie approved

Do not choose a couple system that you’ve written about for another assignment either in this class or any others. To access the movies, visit your local library or view them online through Netflix or another online video service.

Once you have selected a case or viewed the assigned or approved film, begin writing your paper. Be sure to cover each of the following topics:

Theoretical Principles – Describe the theoretical principles about couple therapy that are central to your work with couples? What are the fundamental presuppositions that guide your work, i.e. attachment and connection, differentiation, cognition/behavior, or strengths or internal resources? As an example, EFT holds adult attachment as a key theoretical lens for its process. In your paper, describe what is needed for couple relationships to exist in a stable, satisfying, and thriving manner.

Personal Model Description – Based on your theoretical principles, describe your chosen model of therapy with couples in detail. It is not sufficient to explain your own beliefs about what works in therapy (e.g., I work with couples to help them focus on their love for each other and mutual goals that they want to work on). You must identify a specific model or set of models that provide the framework for your work with couples (e.g., I use an integration of Johnson’s Emotionally Focused model and Bowen’s model with its emphasis on differentiation). Note: Do not try to integrate all of the models. Choose one or two that you’d like to focus on.

In your description of your preferred model of therapy, describe the key processes that you use to bring about change with your couple clients. You should describe the processes of building therapeutic rapport, assessment, intervention, and concluding therapy. Each of these processes should be substantiated with citations from the literature.

Case Synopsis – Provide a brief synopsis of the selected, assigned, or approved case. Describe the fundamental nature of this couple’s relationship as well as the key problems and interactions that are leading his couple to seek therapy. Additionally, describe any systemic factors that seem to contribute to the problems (e.g., an over-involved parent that prevents conflict resolution) and those that could be used as benefits to the couple (e.g., a strong network of social support for the couple and their relationship). Remember, you’re not writing about the movie; rather, you’re using the movie to develop a case study so feel free to add to the characters’ stories to fully develop your case.

Treatment Goals – Identify at least three specific and practical treatment goals for this couple that are consistent with your theoretical principles and intended treatment model. “Help the couple fall in love again” is not specific, nor practical enough. “Stop the domestic violence and establish a safety plan with the couple,” and “Help the couple engage in mutually selected leisure activities” are specific and practical enough.

Focus on Sexual Issues – As discussed throughout this course, sex is an issue that all couples need to address in therapy. What potential issues related to sexuality might this couple need to work through? Some examples include intimacy, communication about sex, and a specific sexual problem? How do you plan to help this couple identify, discuss, and address these issues?

Specific Interventions – Identify and describe at least four specific interventions that you can use with this case based on the model of couple therapy you have presented. The descriptions of the interventions should include information about the purpose and implementation of the interventions and each intervention description should include specific information/application from the selected, assigned, or approved case.

Effectiveness Research – In addition to describing the model and interventions that you will use with this case, discuss the effectiveness of your therapy process based on existing literature for the selected model(s). Additionally, discuss the indications and contraindications of the model (i.e., for which types of clients is this model most appropriate for use and which cases would be better served by other methods). For example, can this model be used with couples that struggle with domestic violence? Do the clients need to be highly intellectual? Are there any reasons why this process of therapy should not be used with certain couple situations?

Attention to Diversity – Discuss the issues of gender and diversity as they pertain to your use of this model.

Strengths and Weaknesses – Discuss the strengths and weaknesses of your use of your model of couple therapy. This discussion should focus both on the general strengths and weaknesses of your approach and on the specific strengths and weaknesses of this approach with your selected, assigned, or approved case.
